Child Pugh classification

Child Pugh classification

المعلومة

Child Pugh classification for cirrhosis pts depends upon: E-PABA
Encephalopathy,Prothrombine time, Albumin, Bilirubin, Ascites
know that Pugh is who added Prothrombine time instead of nutritional state of the old Child classification

Recently the Child-Pugh system has been replaced by the model for end-stage liver disease (MELD) score
for assessing the need for liver transplantation. The MELD score is a prospectively derived scoring system designed to predict prognosis of patients with liver disease and portal hypertension. It is calculated using three noninvasive variables—the prothrombin time expressed as international normalized ratio (INR), serum bilirubin, and serum creatinine

Few points
1- To memorize CPS easier look at the 5 points as
: 2 clinical
3 labs
. The clinical points: Encephalopathy and ascitis
The labs are : 2 for liver synthetic function ( Albumin , PT ) and one for liver excretion function (bilirubin)
Note that liver enzymes do not enter the equation.

2- As for the MELD score it came to existance in attempt to exclude the clinical parameters (encephalopathy, and ascits ) out since these are somewhat subjective and what you may call mild ascitis others may call massive ascitis!! or what you may mild as early encephalopathy others may call it normal.
Still MELD scopre is not perfect and many things (out the MELD equation) are frequently considered : the presence of variceal hemorrhage, hepatorenal syndrome or hepatocellular carcinoma
