2 : Comfortable at rest; dyspnea on ordinary activities .
3 : Less than ordinary activity causes dynpnea, which is limiting .
4 : Dyspnea present at rest; all activity causes discomfort .

تصنيف نيويورك لقصور القلب :
1- يوجد مرض قلبي دون زلة تنفسية مؤثرة على الاعمال الاعتيادية
2- شعور جيد وقت الراحة , ولكن الزلة تحدث بالاعمال الاعتيادية
3- اقل الاعمال الاعتيادية تحدث زلة .
4- زلة تحدث بالراحة وكل الاعمال تحدث شعور بالضيق

this classification is a Functional classification . The classes (I-IV) are:

Class I: no limitation is experienced in any activities; there are no symptoms from ordinary activities.
Class II: slight, mild limitation of activity; the patient is comfortable at rest or with mild exertion.
Class III: marked limitation of any activity; the patient is comfortable only at rest.
Class IV: any physical activity brings on discomfort and symptoms occur at rest.

as opposed to the other classification by the American College of Cardiology/American Heart Association ACC/AHA working group who introduced four stages of heart failure:

Stage A: a high risk HF in the future but no structural heart disorder;
Stage B: a structural heart disorder but no symptoms at any stage;
Stage C: previous or current symptoms of heart failure in the context of an underlying structural heart problem, but managed with medical treatment;
Stage D: advanced disease requiring hospital-based support, a heart transplant or palliative care.

What do u think the best treatment for the 1st and the 4th classes should be?

stage IV should be scheduled for a heart transplant with syptomatic treatment with diuretics and antiarrhythmiatics (or implanted intraventricular defibrillator(
