when I see patients with increased ECFV or decreasedECFV,I first ask:
How might the sodium control mechanisms be impaired?
ECFV overload can be viewed as a state of having too much total body sodium .ECFV depletion cab be viewed as a state of having too little total body sodium.The diagnosis and treatment must focus on finding and correcting the faulty sodium control mechanism .

When I see patients with hyponatremia or hypernatremia ,I begin by asking:How might the water control mechanisms be impaired?A faulty water control mechanism result on too much water relaative to sodium in cases of hyponatremia and too little water relative to sodium in cases of hupernatremia.
The diagnosis and treatment must focus on finding and correcting the faulty water control mechanism.
